☞STUDENT
HANDBOOKS will be given to students on the first day of
school. It will include the following inserts:
- Student
Registration Form
All students
need to fill out the Student Registration each year as emergency
contact numbers, etc. changes from year to year. Please fill
out both sides and return to the school the following
day. If you check your e-mail regularly, include your address
on the registration form. E-mail may be used by teachers
and/or the office to contact you regarding grades, lunches,
etc.
- School Insurance
Form
Sign and return
whether or not you purchase the insurance. If you do
not wish to purchase, write your child’s name and “DO NOT
WANT” on the envelope.
- School
Pictures
School pictures
are scheduled for Monday, August 25th. Information
will be sent with your child.
☞SCHOOL
PHYSICALS AND IMMUNIZATIONS
K-12 students
are required by law to present evidences of their second MMR.
In addition, they must provide proof of vaccination for chicken pox
or documentation of the dates the student had chicken pox.
Parents or guardians of a student in kindergarten, seventh grade or
child entering school from another state are required to provide
proof that the student has completed a physical and vision
examination performed on the student named. Students
participating in the athletic program are required to pass a
physical prior to participating in any athletic
activity.
Each school in
Nebraska is required to keep on file the immunization history of
the students enrolled. Parents or guardians must present one of
three documents to the school to verify immunization
status:
- An immunization
record showing that the child is protected by age-appropriate
immunization.
- A statement
signed by a physician that the required immunization would be
injurious to the student or members of the student's family or
household.
- An affidavit
signed by a legally authorized representative stating that the
immunization conflicts with the tenets and practices of a
recognized religious denomination of which the student is a member.
A student may be provisionally enrolled in a school in Nebraska if
he or she has begun the immunizations series and continues the
necessary immunizations as rapidly as is medically feasible. If the
student does not continue with the immunization schedule, he or she
will be sent home.
- Vision
Evaluation is required for students entering kindergarten (or first
grade, if not enrolled in kindergarten) to provide evidence of
vision evaluation within six months prior to entry. This
requirement also applies to out-of-state transfers. Go to web

☞CLASSROOM
SUPPLY LISTS
Prague Public
School does not identify or require students to bring specific
items to classes in which they are enrolled. Teachers do,
however, provide a suggested list of items that the student or
parent may consider bringing to assist them in recording and
organizing classroom materials. Parents are encouraged to ask
the teachers for the suggested supply lists of the classes their
child(ren) will be enrolled.
☞HOT LUNCH
PRICES HAVE CHANGED FOR 2008-2009
ALL LUNCHES MUST
BE PRE-PAID.
Lunches include
one milk (white or chocolate). Extra milk is .25
each.
Lunch prices for
2008-2009 school year are:
KINDERGARTEN
THROUGH 12TH GRADE: $2.50 EACH
ADULTS: $3.50 EACH
- When sending
money, please indicate how many lunches you are paying for and if
paying for more than one child with one check, write number of
lunches paid for each child! This will ensure that you are given
credit for the correct number of lunches.
- Students will be
offered 3 choices of entrées on most days; one choice each day will
be a peanut butter sandwich. Students will need to choose an entrée
the day before they eat and will only be allowed to eat the entrée
they selected for that day.
- Enclosed is the
Free and Reduced Lunch application. If you wish to apply,
please return form on the first day of school or as soon as
possible. Participation is encouraged in the Federal Lunch
Program as it is a factor in State Aid to schools.

Central. The school does not provide
transportation.
